- Applicants must submit certified transcripts evidencing all prior academic course work including post-secondary education
- Applicants must complete and submit the following medical information:
- a Physician’s Evaluation Form and a Medical History Form completed by a physician, indicating the applicant’s fitness, mentally and physically to pursue a college level study program
- Documentation of MMR (measles, mumps and rubella) immunization
- Proof of immunization for meningitis and hepatitis B, or a signed waiver indicating the applicant’s informed decision not to be vaccinated
- Applicants must provide proof of medical insurance that complies with the requirement of University policy

- UWF offers a payment plan with up to five installments for the fall and spring semesters. The summer semester has a payment plan with up to four installments. The number of available installments is determined by the date of which applicant enrolls in the payment plan
- An account balance of $300 or more is required to be eligible for the payment plan
- Late payments will result in the appropriate late fees assessed to student accounts – tuition $100 and housing $50
- A one-time $15 enrollment charge is assessed in addition to the first payment
