They don't release any forms. You must register and create an account on the official websites of SAT and TOEFL, pay the exam fee, select the test date and center, that's it. Check college board website and ets.org/TOEFL.

BS is the graduation degree just like we have BE, B.Tech BA B.Sc, BS means Bachelor of science, but we have distinction in names between professional (B.E B.Tech) and non-professional courses ( B.Sc BA) they don't have that type of distinction. Yes, you have to give SAT/ACT or their local entrance test and TOEFL or IELTS is compulsory for non-native English speakers.

The syllabus of TOEFL has four sections:
Listening, Reading, Speaking and Writing.
1. Reading measures the ability to understand academic reading matter.
2. Listening measures the ability to understand English as it is used in colleges and universities.
3. Speaking measures the ability to speak English.
4. Writing measures the ability to write in a way that is appropriate for college and university course work.
You can buy books or take some coaching for it too.

TOEFL has four sections namely reading, writing, listening and speaking. There is no particular subject or syllabus you need to refer to. It is all about practice. Refer to TOEFL books and downloadable materials and practice improving your comprehension, paraphrasing passages etc. Read short articles, news, journals regularly. Get yourself acquainted to reading and answering questions on a computer, hence it is important to take regular practice tests.

There is a TOEFL scholarship program named for India, you can check the ETS website for the same. They have certain requirements or score which you need to have in your TOEFL to be eligible for the scholarship. Other than that, a university can give you a scholarship if you have a very good profile including TOEFL scores.
