You can get all the information through bulletin of TOEFL. You need to apply online through its official website. The fees for the exam is around 12-13k INR. The examination centre list is given on the official website that includes all the major cities of India. The exam is conducted throughout the year. The best time for giving the exam is around October and it generally requires 3-4 months to prepare for the exam.

There is no eligibility, but a student must be in their class 11th or 12th. You can take TOEFL anytime during these two years or even after.

TOEFL is not a hard nut to crack it's easy to get through provided that you have proper strategy. TOEFL measures skills in four modules; Listening, Reading, Writing, and Speaking. It depends on the country. Decent US and Canadian universities would require a GMAT score. However, UK does not require a GMAT score. I'm assuming you are looking for MBA programs with 2-3 years of experience.

The application process should start a year before the planned intake. You must finish the application submission by 2017 latest by December, 2017 depending on university websites. The admission windows will open after July/August 2017. You must keep checking university websites for updates.
You can take SAT and TOEFL either during class 11th or before your boards. It shouldn't hamper your board exams, so schedule wisely. You can also apply to colleges, while in the final semester.

As for reporting, the scores will be reported to the selected university everytime you take the test. However, the best score from both the attempts will be considered by the universities. It is very much similar to the concept of betterment exams in India.
