Yes, you can surely apply for any University and by clearing the CET for JMC in IPU or at DU, you can take admission there.

If we see previous year cut-offs, chances are really less for JMC, but other colleges of Delhi University like LSR might give you admission with this percentage in 2nd-3rd cut-off. To check previous year's cut-off of Delhi University, follow the link given below:

No, there is no separate admission form for JMC it is all included in Delhi university under graduation application. Online application start from this week. Stay in touch with Delhi University website.
