For a government seat, it's not possible, but I have seen some people gain addmission in bums without qualify NEET exam but they also face problems with the university exam and unfair admission.

With 199 marks in NEET, you may get admission in BAMS or BUMS. You only have chances of getting admission in a private college because government seats are highly competitive due to low seat intake. Especially if you belong to the general category, you might be able to get admission to a private college in your state depending on the available seats and category-based cut-offs, but your chances are still quite low. The AYUSH cutoff rank for BUMS at  Government Nizamia Tibbi College in the year 2024 was 157239 for generall all India. The institute accepts scores from both NEET and AYUSH for admissions in this course, and only the AYUSH cutoff is available. The cutoff was 160434 in 2023 and 102099 in 2022 for the same. You can visit the official website of the institute to get category-wise cutoff information. Seeing the cutoff, it can be said that a decent score in the entrance exam will be enough to get admission to this college.

Yes, Urdu is generally considered compulsory for admission to a BUMS (Bachelor of Unani Medicine and Surgery) course at Ayurvedic and Unani Tibbia College as most of the teachings at this institute are conducted in Urdu only and Most Unani medical textbooks and lectures are written in Urdu, making proficiency in the language crucial for understanding the curriculum. Students need to have studied Urdu in their 12th in order to apply for this course. In some cases, Arabic or Persian may be accepted as an alternative to Urdu depending on the college policy.

Having 109 marks in NEET, admission to a BUMS (Bachelor of Unani Medicine and Surgery) course in Karnataka may be difficult, but not out of the question, provided you are in a reserved category. Admission to BUMS courses in Karnataka is usually based on the NEET score, and all colleges have separate cutoffs for general and reserved categories. Private colleges may have lower cutoffs than government colleges. Given that you come from a middle-class family, be sure to pursue both private and government institutions that have affordable tuition. Monitor the cutoff trends in each institution.

In Maharashtra, a majority of the Bachelor of Unani Medicine and Surgery (BUMS) institutions make Urdu an obligatory subject either in 10th or 12th grade, as instructed by the Central Council of Indian Medicine (CCIM) guidelines. There are some institutions, though, which provide admission without Urdu if you take a bridge course in Urdu in the first year.
