The cut-off marks in NCHMCT-JEE exam could be around 172 rank for general category and could be lower than the general category, like in the range of 100-150 rank for SC/ST/OBC. Placement wise, if you go for new NIT or IIT (new), there is very less scope. I think you take a 1 year break and improve your rank. Then, you can get easily any old IIT.

If final year subject is Microbiology, you can do M.Sc in Microbiology, Biochemistry, Life science, Biotech, etc.
