Offline procedure:-
Applicant can visit the nearby office of sub-divisional magistrate/ office of Tehsil or Revenue department and can get the application form from there. After getting the application form, applicant needs to fill it properly in a prescribed format and attach the required documents in it as given below.
1) Copy of Ration card.
2) Attested copy of State school Leaving Certificate( SSLC)
3) Attested the fee stamp worth of Rs 5/-
After attaching all the above required documents, applicant needs to pay the fee and submit the application form in an office. After submitting all the documents in the concerned office, the departme
