There is no form filling for admission for b tech in KU. All formalities will be done when you will come in college after counselling of JEE main rank.

You haven't mentioned in which course you are interested. Assuming your interest is MBA it's eligibility is:
- Candidate should be a graduate/post graduate minimum 50% marks aggregate (47.50% marks for SC/ST) from any recognized univ
