For MBBS, BDS, BHMS, BAMS, BVSc, B Sc Agriculture, Nursing, etc (Medicine) you must clear state/national level medical entrance examinations. Other options include B Sc in Sciences, BioInformatics, BioTechnology ad so on.

For Engineering programmes, I mean BTech/BE you must prefer +2 with Physics, Chemistry and Mathematics. For medical stream preferences, Physics, Chemistry and Biology can be your choice for +2. If you wish to be an accountant, +2 Commerce stream may be preferred. Other than that we have the arts stream for +2.

Aim for a B Sc in Physics followed by Post Graduation in Astro Physics/Physics with a PhD. A few IITs and IIAP offer Integrated MSc PhD programmes.

If you have opted for Mathematics at +2 then Engineering, I mean B Tech/BE may be an appropriate track for you. Besides you may also opt for B Sc in Sciences or Mathematics in case you happen to miss the engineering track. If you had Biology during your +2, you nay opt for MBBS / BDS / Ayurveda / Nursing / Agriculture etc, I mean Medicine. If you were Commerce group, then you may opt for B Com followed by M Com/MBA track.

Chances are really tough. In case you are from general category, then almost no chances. If you are from a reservation category, then still some. Chances, if you get a really good percentile

I hope you have 50% aggregate in +2 with PCM. You must clear national/state level entrance examinations with a good score as applicable to your institute/university. For admission to IITs and IISc you must clear JEE. Many private deemed universities conduct their own aptitude tests.
