To get your original DNB pediatric pass certificate from 2020, you do not need to apply separately. The National of Examination (NBE) usually sends a provisional pass certificate and marksheet by mail to your registered address within a few weeks or months of the result is declared. If you have not obtained it, you can contact NBE directly to help through their website or candidate care aid.

To pick best DNB hospital for learning, focus on high patient volume, plenty of hands-on cases, a supportive seniors/faculty, fewer co-trainees for more individual attention, and a less toxic environment, preferring government or mission hospitals over some corporate ones for better primary exposure and a good mix of tertiary/secondary care. Always visit hospital, talk to current residents, and check bed strength (400+) for better exposure.

In simple terms DNB focuses heavily on hands on clinical skills in diverse hospitals (often private) with a tough, centralized exam but MD/MS gives more structured, research focused learning in medical colleges with university specific exams leading to slightly different career paths but both are recognized specialist qualifications.
Think of DNB as more "real-world, fast-paced" training and MD/MS as more "academic, textbook-heavy" with strong mentorship but DNB is gaining ground for its clinical depth.
