Hi, you should have minimum 55% in 10+2. Application forms are available online for the session 2018-19.
For more details please visit http://jmi.ac.in/
You can fill application forms for as many courses you want. You have to clear an entrance exam to get admission.

If it is general paper, it will come in Hindi and English both, but if it is honors paper, then it will only come in English, and you have to answer in English too.

No, you cannot select Hindi instead of English as English is a compulsory subject to add in the best 4 percentage. You can add your best 3 subjects with English and then calculate your best four percentage.

Eligibility for MA in Hindi is graduation with 45% or more and
MA in Hindi from JNU is conducted through the entrance mode, then the merit screening will be done. So on the basis of your entrance performance, you get admission in MA in Hindi in JNU.
