So if you're planning to get into Post Basic B.Sc. Nursing at CIHSR, you basically need to have done GNM and be a registered nurse. They don't have a separate big entrance exam or anything most of the time — it's usually based on your marks. Forms come out somewhere around April to September. After that, if you're selected, you just have to give your documents, pay fees, and do a medical check. That's pretty much it.

No specific percentage required in 12th grade.
Eligibility simply states you must have passed 10+2 (Arts, Science, or Vocational) with English, or equivalent from recognised boards or NIOS
This isn't currently offered at Ascent Tezpur, but typically requires 45% aggregate in PCB + English with relaxation to 40% for SC/ST elsewhere
