Candidates applying for MPharm programme at Uttar Pradesh University of Medical Sciences have to pay an application fee of INR 3,000. There is a relaxation of INR 1,000 for SC, ST candidates. The application fee is non-refundable. Candidates have to pay the fee in the form of DD in the name of "Finance Officer, UPUMS" payable at Saifai.

Candidates seeking admission to the BPharma programme of CSJMU must submit the following documents at the time of admission:
- Class 10 Mark sheet & Certificate
- Class 12 Mark sheet & Certificate
- DPharma Mark sheet (for Lateral Entry)
