Different colleges have different requirements but most importantly any good or top ranked college will prefer highly academic students, so you need a good percentile. Secondly, you must take IELTS or TOEFL. To study in US, you must additionally take the SAT or ACT. Good bands in these tests and strong extracurricular activities are also required. Admission process is online and very simple. You can begin by checking university websites, get familiarized with the different requirements, fees, etc. and prepare accordingly.

Typically you must be a good student, good grades during your 11th and 12th is required. You must take the IELTS very importantly. No other tests as such are required unlike in the USA. However, to apply to universities in the UK you have to apply via the UCAS common application web portal. You may type UCAS on google and find out. At an undergraduate level, you need to have a good academic record and also, finance to sustain abroad for three years. At PG level too, academics plays an important role but finance takes a little back seat.
However, there are many nuances and hence it's best to approach an agency dealing with sending students abroad.

46% is a poor score. Not easily acceptable abroad. Even if a low grade college does accept, you won't get the visa. And, if you do get the visa, ask yourself if studying in a low grade college abroad is worth it. Also, you should know that UK degrees are not recognized in India anymore so think twice before investing your money and time. Check private institutes in India or do a PG Diploma course here, try to secure at least 55% to 60% and then plan to study abroad.

Please narrow down your choice based on factors like your budget, your profile and admission requirements of the universities, climatic and language preference etc. Based on your BE grades Germany and Canada looks next to impossible. You may if you have a competitive profile. High academics and extra-curriculars are a must. Besides, it will depend on the university or the college if it finds you competitive before offering you a scholarship. Since scholarships are offered by the colleges or universities abroad you are advised to check the institute website and find out about financial aids given to international students. Also check the DAAD India website for schalarships to study in Germany and the British Council website to study in UK.
