Admissions into universities like Stanford, Harvard, etc. are not done basis of the scores alone. 95% is a great score. But what about your 10+2? And, also do you have extracurricular activities? Have you done any project and research work? Have you participated actively in sports and community services? If the answers are 'yes', then perhaps chances are there. Visit Stanford website and read about the admission requirement. You have to prepare yourself from now onwards. And, JEE Mains are not considered there. You must take the ACT or SAT.

There is no set criteria but your entire academic record from 10th onwards is very crucial. Both your academic performance and extra curricular activities will play an important role during the admission process in these top ranked and selective universities. 70% or more and SAT score of 1350+ is suggested.

Almost all US universities require 16 years of education. That is four years of education after high school/junior college.
If you have a three-year degree from an Australian, Indian, New Zealand university or such similar three-year degree programmes, you will not be eligible for admission in most of the MBA programmes. You would require a professional four-year degree like engineering or any other course.

If there is no counselor at your school, you can name your school principal or any other school official, who has overseen or evaluated your academic progress throughout. This same person will be required to complete the school report on the recommenders and FERPA section.
