Don't worry about the things you can't really change. Although, you will be at disadvantage, because of low scores in the past. You can offset it with scoring higher percentile in CAT and get admission in one of the top 10 institutes. All the best.

After 12, you need to complete graduation in any field, say commerce, arts or science.
After that, you need to appear management exams like CAT, CMAT, MAT, CET, XAT, IIFT, NMAT, SNAP, GMAT, TOEFL, IELTS and GRE (if you want to pursue MBA outside INDIA).
Scoring 99 percentile to get calls from IIM should be your target.
