No it is not all mandatory to take a joint M.Sc PhD after qualifying for IIT JAM. Although many students choose this, you can just apply for M.Sc now and if you feel apply for PhD later.
For GATE also the similar thing is followed.

The total fee for MSc at Manipal Institute of Regenerative Medicine is INR 8.26 lakh. The total fee includes a refundable caution deposit of INR 10,000. The University of California Riverside Campus requires GRE for the Biophysics graduate programme only. Other than the programme mentioned, the GRE test is optional. Although, the students are generally suggested to take GRE for making their application stronger and raising chances to be admitted to this university.
A strong GRE score for the University of California RIverside Campus is considered above 310. However UCR does not have a sepcific cut-off for GRE.

Both are acceptable, but MS + 3-year MCh is more traditional and widely acceptable with the strong surgical background. The 5-year direct MCh (after MBBS) is a new one and is offered in fewer colleges. It is time-efficient but may lack general surgery exposure. Further, the decision is based on career goals and performance in exams.
