Since you already have 4 heavy subjects to study, you should opt for a light subject like physical education if you are planning to take some coaching. If not, then go for one of Computers, Economics or Engineering Drawing depending upon what you want to do after 12th.

Appearing in entrance is mandatory to become a doctor. You need to appear national level common entrance examinations for admission into MBBS

MBBS doctors are posted as Junior Residents or Medical Officers in Sub Centres just after MBBS. or you can start private practice.
