You have to apply to the universities through common app or individually. Your SAT/ACT scores are sent directly by the respective agencies. With your application you will be required to submit your high school transcript, letter of recommendations from principal/counselor and teacher, an essay or Statement of purpose, etc.

Apart from the test mentioned by you, there is AP programme in which you can choose the subjects and take credits before joining the course in USA by clearing it with good score.
The scores in ACT, SAT or TOEFL/ IELTS depends on which universities u plan to apply. Higher the score, better are the chances of scholarships. For SAT any thing above 1280 is considered good.

If you are particularly aiming for IVY League, then you have to be damn good in your academics and ECs. To get admission in US universities, you have to take either SAT or ACT. Both these tests are for undergraduate courses.
As an Indian student, I would suggest you to register for SAT exam as it is favorable for Indian students and due to large number of test takers. You need to understand the structure of the exam. It just measures your basic mathematics and strong language basics.
For Indians the mathematics section seems to be pretty easy, but as English is not our first language, the language section creates a bit complication.

A score of 26 is a great start. Most good universities in United States generally look for student scores between 28 and 30. While the very best schools often expect a 33 or even higher. Always prepare well for it.

If you are in class 11th, prepare either for SAT or ACT and IELTS or TOEFL. You need not take GRE (which is taken for Masters in USA). You must register yourself on the official websites of the respective test administrators, create an account and schedule a test date. You will find information on syllabus, test format and online test prep materials too.
