I will suggest you not to pursue any Bachelor course from distance learning, as they have less value as compared to a full time degree. If you are able to afford regular college fee then take admission into that, BCA is a practical course which requires proper guidance as it includes Programming Languages, which are not understandable without any proper guidance. And, companies also avoids candidates with distance learning degrees.

It is preferable but not mandatory. In West Bengal, under MAKAUT affiliated Institutes previously you could pursue BCA provided you completed your +2 with any one of Mathematics/ Business Mathematics/Computer Application as compulsory subject with others. But now they allow candidates for BCA only those who have 50% or above in H. S from any stream. But some Institutes in India, the eligibility criteria is Mathematics as compulsory subject in +2 level.

Repeated practice can improve that. BCA Maths is quite easy. So don't worry about it. If you really have interest to do BCA then go with it. But, if you want to pursue MCA Maths plays an important role. Competitive Exams like NIMCET AIMCET and so on gives importance to Maths. So, whenever possible practice Maths. Or else, join any institute which provides BCA+MCA integrated courses. But, if you want to pursue MCA from reputed institute like NIT, then Maths is important. Note that, NITs don't provide integrated courses.

After 12th, you can apply for BCA. Generally, the eligibility criteria for most colleges is that a candidate needs to score at least 45% in +2 to be eligible for admission into BCA. If you want to learn programming languages which you will be studying in your BCA course then you can check out this app "BCA - Course Programming" which is specially designed and developed for BCA students and is available on Google Play Store. It has programs and assignments of languages like C, C+ and Java with their outputs. You'll be studying these languages in your 3 years of BCA.

It is not necessary for the student to have computer, as a subject in intermediate. He/she can still pursue the BCA. All what you need is to score at least 50% marks in intermediate to be eligible for BCA. Hence, there's no problem of subjects or streams, if you want to join BCA course.
