Hello,
An overall evaluation is done for selecting candidates at IMI Kolkata. However, with 75 plus percentile in CAT and decent scores in undergrad, you have fair chances of converting. IMI Kolkata has had an excellent track record of hundred percent throughout the years. Eminent faculty members from all over the world come to teach the students at IMI Kolkata. It has world class and ultramodern infrastructure. Students from different backgrounds from all parts of the country come to study in this college. Hostel facility is also present. Many live projects also take place which gives students a new perspective on the corporate world.

There is no need of filling separate application form as CAT score is acceptable in every institution, so there is no need of separate application form.

Yes, it is a compulsory process of any IIMs. Your CAT score is not enough even if you get 99 percentile.

Your academics scores are decent.
All the IIM A and B have a different CAT cut-off.
Like IIM-A's minimum 80%ile and the sectional cut-off is also given weightage along with class 10th, 12th and graduation marks, and work exp. Similarly, in IIM-Bangalore and Calcutta, the minimum is 90%ile and other factors matter too.
