1)The first and the foremost, you need to fill an online application for Delhi University mentioning all the courses that you wish to seek admission in. Though you can also go for offline application, but it will be convenient to fill it online. You will also be required to fill your class 12 board marks subject wise.
2)Next, wait for the cut-off lists.
3)For the B.Sc courses, the cutoffs are to be calculated as
For Physics, Chemistry, Zoology, Botany, etc your cutoffs are calculated as per your percentage in PCM
For Mathematical Sciences, Computer Science, Mathematics and Statistics, cutoffs are calculated as the aggregate of marks in mat

Actually after graduation admission in various programmes /courses depends on percentage and grades. But most important is individuals interest. If you ask me personally, as you are a PCM student, I will advise you to go for Masters programs / research in physics/chemistry or mathematics from a good University/ College. In India we have a huge demand of academicians /teachers. More over future of teachers is very bright in India.

It is really difficult to say that Integrated MSc with Phd is available or not. Because there are a lot of private universities who are offering these courses in integrated manner. that is Masters degree along with Phd Degree at a stretch of 5 years (2+3) but it will be difficult to say that if that courses are accredited by national boards. because normally the boards accept regular courses.
They consider integrated courses as distance courses and hence have no equivalency with regular ones. hence it would be wiser if you pursue you MSc and then apply for Phd and complete Phd after completion of masters only. again you have asked for
