The procedure and requirements are different for different universities. Normally, the procedure is online, you must submit the online application form within the specified deadline. Upload your academic transcripts, test scores such as SAT or ACT, IELTS or TOEFL, SOP, LOR, CV, etc. Check university specific requirements prior applying.

Hi Sourav,
It is difficult and depends on the university. You should start emailing some universities and find out. Many universities can accept the candidature after a WES evaluation of the degree. Top ranking universities like MIT and Harvard also accept 3-year degrees but they accept a few exceptional students. It is difficult to say since it depends on the admission offices of the university. Your profile must also be a strong one for them to consider.
