Merit list is prepared on the basis of 60% weightage to JEE (Main) / NEET-UG Score [total marks obtained] and 40% weightage to marks in Class 12 / equivalent qualifying examination. Go to the course brochure provided by the college over there they would have specified the format of certificate or the other acceptable methods, or if they will conduct the test and make their own certificate. Thank you.

Yes, you are eligible for giving the NEET exam. You must have studied physics, chemistry, biology and English in 12th.
