If you're interested in some sort of career, plan accordingly. For example, you want to be a chemist, go for a B.Sc in chemistry. Similarly, to be a physicist or if some research drags your interest in physics then be in physics.
If you are opting to settle for better, then, B.Sc chemistry is for you. Physics degree must be accompanied by a Master's degree, but a chemistry Bachelor's degree could do it better in fetching you a job. I, personally, had my uncle go for a job in a pharmaceutical company after he completed his B.Sc PCM (physics, chemistry and maths).

Yes, a combined form if filled first and then, the preferences are asked. One can fill as many subjects as they like.

Hi Avani, first of all congratulations for completing your B.Sc. Chemistry in first class and for next option you can choose preferably after B.Sc chemistry is an M.Sc or MBA. The best would be go for an M.Sc.

I am a Masters in chemistry, and I am pursuing MBA currently. You should understand that what you want to do, you want to do research or you want to join corporate in your future. I tell you one thing clearly, it is very tough to get a job in pure Science stream.
