Follow these steps:
1. Learn all formulae from each chapter
2. Take previous years paper and practice them diligently
3. Manage your speed by doing mock tests
4. Stick to your text books.

2) Appear the entrance test for taking admission in MD/MS course.
3) Choose gynaecology as a specialization in MD/MS.

You have not mentioned the subjects. Now, which course is best for you is something only you will know. What might be best for me may not be good for you, So, instead of asking a third person, analyse your own aptitude and skill set. Figure out what you like studying the most, there could be areas you excel without any effort, select such an area. Once you know what to study you can check for the colleges.

You have to wait till the merit lists are out. In general cut-offs are as high as 80% for B.Com programmes. College prediction is difficult without knowing the merit list.

An improvement exam is recommended for better chances of higher studies. Alternatively there are tier 3 colleges where you can apply if you are not keen to re-attempt your 12th.

You have to score minimum of 60% in 12th and must clear your JEE (Main and Advanced). Recommended subjects are PCMB. For more details please check the IIT-Madras website
