You can definitely prepare for CAT and if you prepare well you might even get 99+percentile. But, even then getting into an IIM will be quite difficult, because they give a significant weightage to your past academics as well. But, there are also some very good institutes at par with the Iims, who do not give that much weightage to your past academics, provided you have a good profile and a very good percentile in CAT.

It's only based on how much you understand the concept and your efforts if you want to get top MBA school you have to get at least 90 persantile in CAT
and more than 600 in GMAT.

Most probably, no. because, for NET eligibility, you need to have a degree approved by UGC or in some cases AICTE. Since PGPM is a certificate-based management programme it can't be used as a medium for getting into NET. Yes, for MBA, graduation is the main eligibility criteria which you have. There is no any much important role of 12th standard.
