ENT, dermatology, orthopedics, ophthalmology, and pediatrics can be good options to opt for in MBBS at government colleges. Do look at the various cutoff percentile of each colleges for a clear report.

The cutoff for MBBS at Maharishi Markandeshwar University is as follows.
For the year 2020 ,the cutoff rank is 162181.
For the year 2021, the cutoff rank is 222350.
For the year 2022, the cutoff rank is 253028.

590 marks is good score in NEET and according to this marks and this rank you can get a government medical college for MBBS. You can try for government colleges according to you.

The MBBS cutoff for Apollo Institute of Medical Sciences and Research, Hyderabad, varies for different categories. With an 82787 rank, you may be eligible for admission through the state quota. Since you have not mentioned your category, it is difficult to accurately answer. However, based on the previous year's cutoff data, you can get MBBS at Apollo Medical College with a 501 score in NEET UG if you fall into any one of the following categories:
BCA: Previously, the cutoff has been around 481. ST (Female) In 2021, the cut-off was around 501. SC (female): in 2021, the cutoff was 481. BCD-CAP-NL (female) cutoff is generally less than 50

1- JSS Academy of Higher Education and Research, JSS Medical College. Fees of JSSMC is INR 6,49,000 for 66 months
2- RajaRajeswari Medical College and Hospital (RRMCH)
Fees of Yenepoya is INR 5,47,000 for 66 months
