It is difficult to answer accurately in the absence of your category and home state data, but you may have a chance to get admission to government medical colleges (GMC) with 590 marks. According to previous year's cutoff data for GMCs, if you belong to the general category, you need to score more than 600 marks in NEET to get admission to most colleges. Yes, of course, you can open a medical diagnostic lab if you have a valid license. But, you need to consultant doctors for confirmation of the diagnosis report.

With 138 marks, you have very few chances to get into any private medical college for MBBS in Uttar Pradesh. According to the previous year's cutoff data, you may have some chance of getting admission to the Career Institute of Medical Sciences and Hospital, Lucknow. Otherwise, you can apply to private dental colleges in Lucknow, such as Babu Banarasi Das College of Dental Sciences, Sardar Patel Post Graduate Institute of Dental & Medical Sciences, and Saraswati Dental College, for a BDS degree.
