For operations management, you can target for NITIE-Mumbai, SIOM-Nashik and NICMAR-Pune apart from IIMs. You need to score at least 90 percentile in CAT to get eligible for these institutes.

Operations management is a lucrative career option at Chetana. We have great faculty members & many growth opportunities in this field. In our second semester, students from the operations background also get some exposure to the industry by the way of visiting exhibitions etc. I am not aware of the placement scenario as such. But in general, there are good companies that visit the campus for placing students from all streams.

Yes, every good and established university offers MBA with dual degree specialisation, so accordingly you can choose your specialisation in 2nd year.
