You have to check the same on particular university websites. Please visit university websites such as Princeton, Yale, Harvard and so on and look for the summer internship courses for international students. These programs are offered in many disciplines and have separate application deadlines.

To study in Germany one must complete 13 years of education that is one more additional year of formal schooling or college after 12th.You may enroll into a Bachelor's degree program and complete the first year or check for Studienkolleg courses in Germany.
You must also have strong German language skills since the undergraduate courses are mostly taught in German language. Medical studies are highly competitive and all requirements must be met. Check the DAAD India website for more clarity.

47% is too low for any foreign university, let alone Canada. You can and you should mention your disorder issue, challenges you faced and also overcame on your SOP however it won't help in too many ways since your academics are way below the requirements.
A certain set of standards have to be met. However, there is no harm trying, you can give your 100% and give a try at least. You can try applying to lesser competitive countries such as Poland, Ireland if you wish to.
