But there are few things which should be kept in mind as ICSI has started giving very fewer results.
1) Patience- It is the key to success at least for CS executive exam.
2) Mental and physical strength-Exams are conducted consecutively without breaks. Your body feels drained after first two exams.
3)Confidence

Hi,
How many exams you need to prepare completely depends on the type, of course, you are applying to. For all undergraduate courses in US and some in Canada, you need to prepare for the SAT. For MBA courses GMAT scores are a requirement; similarly, for MS courses GRE scores are relevant. Along with all these exams, you have to take either IELTS or TOEFL.

Acknowledging that you are really concerned about the improvement, I shall appreciate that you know the British Council Learning and shall surely benefit if you do a course with them.
Besides, I shall also suggest that you should initiate Reading of Simple texts, not newspapers, simple texts like stories and small write ups that make you see new words and vocabulary.
Do listen and practice speaking.
You can start some training or coaching for remedial grammar because in India EE learn English as second language and always need to improve our grammar.
