Orenburg university does offer English Medium though there are many other universities which have more experience in teaching international students in English.
You need to be aware that in Russia you will be taught Russian language along with your medical curriculum in English. It is mandatory.

There are many universities in Russia which have started English Medium of education. But you need to ensure that the university is listed with MCI and moreover it has more experienced faculty who can teach in English.
The selection of university will also depend on your affordable budget.
Some of the best universities offering English Medium are Russian National Research Medical University, Moscow, First Moscow state medical university, Belgorod State University, Dagestan State Medical University. In CLAT exam, English portion covers 40 marks and 30 is a pretty much good score. All you can do is go through past year CLAT question paper that will give you a better idea how to strategies your planning.
