Yes, DM (Doctorate of Medicine) is higher than MD (Doctor of Medicine) in the medical field.
MD course is done after MBBS. It makes you a specialist in a broad field like General Medicine or Pediatrics.
DM is done after MD. It makes you an expert in a very narrow, high level field like Cardiology (Heart), Neurology (Brain) or even Nephrology (Kidney).

No, DM is not equivalent to a PhD. These are two types of PG courses in Medicine and health sciences with different points of focus and purpose.
DM is clinical and focused in orientation with the focus being on the care of patients and practice of medicine.
But PhD is focused towards research and contributes to the body of knowledge in science.
