In USA it may be waived off depending on the university and your profile. But in UK it is compulsory no matter how good your academic grades might be. You must take IELTS for UKVI to study in UK.

Hi Abu,
There is a TOEFL scholarship program named for India, you can check the ETS website for the same. They have certain requirements or score which you need to have in your TOEFL to be eligible for the scholarship. Other than that, a university can give you a scholarship if you have a very good profile including TOEFL scores.

To study MBBS abroad student requires an eligibility certificate from Medical council of India (MCI).The eligibility certificate can be downloaded from the MCI website. Students who have 50% marks and above in Physics, chemistry, biology and English are eligible to apply for MBBS studies abroad.
TOEFL is nothing but a language proficiency test. If English was not your medium of instruction in your education, you might have to appear for TOEFL or IELTS. This is not compulsory if English was your first language or was used as the language of instruction in your school/college.

They don't release any forms. You must register and create an account on the official websites of SAT and TOEFL, pay the exam fee, select the test date and center, that's it. Check college board website and ets.org/TOEFL.

Fee waivers are generally not applicable because these are tests. If you do score good marks in these tests you may avail scholarships to study.

They will consider you if you clear your SAT and TOEFL. If you are confused you can apply towards what is called and undeclared major in USA. You can apply to a Bachelor's degree but not declaring any specific concentration or specialization. You can later choose after you have explored the courses, subjects and also you will know your aptitude an interest. This will also depend on your test scores and academic performance.
