Hi, scholarships are on merit basis, if you have a very good profile chances are there of obtaining a scholarship. You can also opt for an education loan from Indian banks. Another option is to check trusts and organizations providing financial assistance such for higher studies abroad, some include Fullbright Nehru Scholarship to study in USA, Aga Khan Scholarship etc.

Keeping money factor in mind, you should go for Canada for your undergraduate, as Canada is cheaper than USA. It is also good for your undergraduate. It's curriculum for the particular is nice.

A very ambiguous query. What is it that you want to study? What are your grades? It is also difficult to list down the procedure for each and every university in every country you mentioned. Remember, the admission requirements differ accordingly. For a head start, prepare for IELTS or TOEFL and also GRE for US.

Hi,
University of California Berkeley is among the top ranked universities. You are advised to take TOEFL instead of IELTS. GRE is not required for this programme in UC Berkley but you may need to submit a portfolio and have excellent academic standing. For more details please checj their webpage.

Scholarships are provided by universities or external sponsors, most information are obtainable from university websites. They are given on merit. Some scholarships may ask you to submit essays etc. While for some you will considered for it at the time of application by default. You can go about checking university websites for specific instructions and information.
