Go to official website of BITSAT, then go to academic section and click it. In next page, got below and click important forms (anti ragging + medical certificate). From there click on medical certificate and it will get downloaded.

BITSAT Eligibility Criteria for B.Pharm (Hons.)
Candidates must have passed 12th examination from a recognized Central or State board. Candidates must have Physics, Chemistry, and Biology as subjects in Class 12. Candidate must secure minimum aggregate of 75% marks in Physics, Chemistry and Biology subjects.
