You can apply but admission chances with backlogs is slim. You will also put yourself at high risk of visa rejection. However, if you have a convincing SOP and LORs you may have a chance. Write to the universities before applying.

You must check the tuition rates on university websites, if it includes health coverage and additional student fee. As for Ph.D admissions, you can get in touch with the department specific advisors, enquire about stipends, assistantship, scholarships, international student awards, etc.
