Amongst the options, USA and Canada are English speaking countries and excellent for studying Medicine. Besides the country, more important is the college or University you study at. Germany is also home to prominent medical universities. But the education is in German language completely. Russia has been a choice of many Asian and African students to study medicine since many universities in Russia have started providing support in English medium and Russian is taught gradually. Only if you have more number of backlog, then Canada will not accept more than 8 backlog. Germany will accept, if you would have cleared it within the course period.

This will depend from university to university, for UG programs up to B1 is normally required. For PG, it may not be mandatory, since classes are held in English language mostly.
