While mostly migrations (if allowed by university) are permitted in 2nd year, but see if you university allows on 3rd year also. You can also talk to VC or approach court but nothing can be done if seats are not vacant in the college. Try giving an undertaking for attendance maybe that works.

Both medical and paramedical courses are good. Medical courses are difficult in comparison to paramedical courses. So if are interested in paramedical courses then you can opt for that course.
