Institutions would mostly require a good class 12th score (70-80%+) and few may (NUS, NTU) also require an IELTS score of 6.0. Hope this helps.

I would suggest you to write directly to the universities you are interested in as most of them require completed Bachelors degree + 2-3 yrs work experience, TOEFL/ IELTS and references.
Since you have 23 ATKTS you need to check with them how they evaluate it. SIM, SP Jain, NUS, NTU, Concordia etc are some of the universities you can look for.

Irrespective of the country, there are several factors which can affect visa chances such as irrelevant course choice, not meeting admission requirements, no clarity over one's intention to study abroad, hints of immigration etc. There is a useful discussion on visa rejection here, you may go through the same:
