Yes, you can enrol yourself for Ph.D on the basis of the provisional certificate which can be obtained from your current college. it will be even better if you can contact your college for the detailed information about this.

No. There won't be any problem. The degree completion date will be required for further form fillings and prof of degree completion.

As per the question, you completed 2-years of your education and dropped 5th and 6th semester. Details of the year in which you have dropped the course is not mentioned, I think you will need to speak with the college authorities for reliable information. In case the gap is large, you can continue the course with distance education as the fee is comparatively lower.

You can opt for the course (if you wish to pursue the same course) via distance education. There are so many universities in India that promote education via distance learning or correspondence. Few examples are IGNOU and DU SOL.
