It's offline. The centre will be alloted according to the location and you will have all courses mixed maths, science, aptitude and many other topics will be covered in the question papers. There are books for reference.

There is no such compulsion, however in case you do not attempt all the sections then it would be difficult to get a decent rank.

Yes, you are very much eligible to study BHM course if you have completed your +2 from NIOS board as NIOS board is also considered as a recognised board.
However, below I have mentioned the eligibility criteria for pursuing BHM course.
Kindly have a look at it.
Eligibility criteria for BHM course:
Candidate must have passed +2 level of examination from a reputed Institute
English should be one of the main subject in +2 level
Candidate's age (General and OBC) should not exceed more than 22 years at the time of applying for the course
