You are qualified to do your Ph.D but not quite strong academically. However in USA, there are many options. You have to write to the universities yourself. Visit the department pages, check the Ph.D. information and email the research supervisors. Submit some work if you have, you will also require a strong recommendation network.

The exam pattern may be different for different colleges. Generally, the exam will be subjective type. There will be no negative marking. The cut-off marks required may differ with universities. The questions will be basically from the subjects covered during post-graduation period.
