For getting an admission into operations and supply-chain management, first you should clear any of the examinations like CAT MAT etc. Then, choose a college which is offering these specializations, then get into the admission procedure into that.

This would depend on your interest areas and career aspirations. In case you intend to pursue a career in research / academics area, then MA/ B.Ed is better. However, if you want to move into general management then MBA is better.

Of course, but maybe, good college won't give you response due to very poor percentage in graduation but in life, there are always many choices.
So, just start applying and take XAT exam or TOEFL that will help you to cover your poor percentage in graduation.

These are two different domains. You can either choose Operation and Production Management or a supply chain. If your domain is more inclined to logistics, you can opt for a SCM or if your domain is more in production, procurement, or purchase you can prefer Operation Management. For better understanding, I can guide you better.
