The supplementary application form is more of a personal statement through which you must showcase your motivation, skills, achievements and so on. You need to take and clear an IELTS test with 6 band, not less than 5.5 in each module.

Your 10th and 12th marks, Under graduation GPA (if applicable), IELTS or TOEFL scores
Course based scores -
GMAT scores if you are applying to MBA
GRE scores if you are applying for MS course.

you can send available mark sheets. you need not wait till you get your final mark sheets. based on the available mark sheet you will get admission with condition to complete and then join. it is called conditional offer. It is better to apply n advance to get this conditional offer and with this offer letter you can apply fir bank loan also.
