National Testing Agency has released RE-NEET Self Declaration Form 2026 at neet.nta.nic.in. Students can download NEET 2026 undertaking form PDF along with admit card for June 21 examination. The undertaking form for NEET UG 2026 is an important document that candidates have to carry to the exam centre. Unlike a separate application form, the self-declaration form is attached to re NEET 2026 admit card and can be downloaded from candidate login portal NTA NEET website.
This undertaking document contains candidate declarations, exam-day instructions, space for a photograph, thumb impression and signature. Candidates have to read all instructions carefully and keep the form ready before reporting to NEET exam centre. The NEET UG 2026 re-exam will be conducted in pen-and-paper mode from 2 PM to 5:15 PM.

How to Fill RE-NEET Undertaking Form 2026?
Candidates should carefully complete the following requirements in NEET undertaking form 2026:
- Read all exam-day guidelines
- Paste same passport-size photograph used in application form.
- Put the left-hand thumb impression in dedicated space.
- Cross Check personal details printed on the form.
- Do not sign where the invigilator's presence is required.
Important Note for Students: Failure to carry mandatory documents may lead to difficulties during verification at examination centre. Students must carry their admit card, self declaration form and valid ID proof at the exam centre on June 21.
