After 12th there is a plethora of career options broadly into arts, science and commerce stream. To decide on what is good for you, you need to understand your aptitude and your interest. In order to do this, you need to take a career test comprising of these two components.

As per the CCH rules, You don't have any option of taking up a surgery course as that requires MBBS at least in India. You may look for various other options as compiled by one of the professors here:

Not too many universities in Canada accept international students into their medical programs. And, I don't think there is MBBS. You have to enroll into a bachelor degree program in biological sciences and complete the same with very good grades. That's not all. Post your bachelors, you must clear the medical college admissions test and only then seek entry to a medical school. Becoming a doctor in Canada is a long road. Check if you are eligible to study firstly. Visit university's website for the details. Fees is around $30,000 per year in Canadian dollars.

You cannot study medical without a relevant background, since you had commerce I doubt if you are eligible. You can check for associate degree programs which are of two years before proceeding to a Bachelor's degree. I would advise you to check directly with the colleges.
