You have to clear your 12th with good grades and take the SAT or ACT and TOEFL. Please check the college website, the entry requirements are mentioned clearly. The procedure of application is online, you have to create your account and submit or upload your documents, academic transcripts, SOP, LORs online. You can either apply via Common Application portal or Babson Application portal. See here:

Both the fields are pretty good. You shouldn't think about the scope. Go for the one in which you are interested. MBA operations is an evergreen field. On the other hand, the demand for innovation management is surely going to rise as more emphasis is being given to research nowadays.

You can go for technical courses to add value to your resume. To be more efficient, you can go for PMP certification, Six Sigma certification, R course, Python, Advanced Excel, Edx, Google analytics, Front Office Management and Inventory Management. PMP certification can be done with the experience of 3 years or more. So, you need to have a preparatory course of PMP to take the exam. While both of the colleges are quite comparable in terms of placements, Xavier Institute of Management and Entrepreneurship, Bangalore has a slight edge over ITM Business School, Navi Mumbai in terms of reputation and rank index.
But, if you are from Mumbai, I will advise you to take up ITM, Navi Mumbai as the placements are almost equal for both the colleges and the ROI will be pretty decent for ITM.
The average placement for XIME is around Rs 6.4 lakhs while the average placements for ITM come to around Rs 6 lakhs.

For being an entrepreneur you have to be an all-rounder in the field of marketing, HR, finance, operations, etc. as you are to establish your own business so you need to be more productive and should have all the domain knowledge which would be required to execute your business idea into reality.
