Changing your MD specialisation after joining is extremely difficult and generally not possible. The process consists of leaving your seat now and appearing again for NEET-PG exam and getting your desired seat in the next counselling. In some situations, you may have to pay a bond penalty or notice period fee to the state government if you leave a government seat.

No. NEET-PG is mandatory for all MD admissions in India across all colleges, government, private, deemed, and central institutions. There is no alternative entrance exam or direct admission route for MD. Even foreign nationals of Indian origin need to qualify in the relevant entrance process.
